Treatment Plan
Therapeutic goals and progress tracking
The Treatment Plan section helps you create, manage, and track treatment goals for each client. Each goal includes detailed information about objectives, interventions, target dates, and completion status.
Accessing Treatment Plan
From any client record page, click "Treatment Plan" in the left navigation sidebar to jump to this section.
Header Display
The section title shows the total number of goals:
Example: "Treatment Plan (5)" indicates 5 total goals for this client
Header Actions
Add Button
The "+" button opens a dialog to create a new treatment goal:
- Click the "+" button
- Treatment goal dialog appears
- Fill in goal details (see Goal Structure below)
- Click save to add goal
- Goal appears at top of list
The add button is disabled if:
- You don't have note owner permissions
- The client has exceeded usage limits
Expand/Collapse Button
Controls how many goals are displayed:
Collapsed: Shows first 3 goals only
Expanded: Displays all treatment goals
The button appears only when more than 3 goals exist.
Goal Structure
Each treatment goal includes:
Title
Required field - Brief description of the goal
Examples:
- "Reduce anxiety symptoms"
- "Improve sleep quality"
- "Develop coping strategies"
Description
Required field - Detailed explanation of what the client aims to achieve
Include:
- Specific measurable objectives
- Baseline information
- Success criteria
- Client's own goals and language
Intervention
Required field - Therapeutic approach or techniques to achieve the goal
Include:
- Specific therapy modalities (CBT, DBT, etc.)
- Techniques or exercises
- Homework assignments
- Frequency of practice
Target Date
Optional field - Expected completion date
Use for:
- Time-limited goals
- Session-based objectives
- Milestone tracking
- Insurance requirements
Format: Date picker showing month/day/year
Completed Status
Checkbox - Mark goal as achieved
When checked:
- Goal shows completed indicator
- Remains visible in the list
- Can be unchecked if goal regresses
- Timestamp recorded (for internal tracking)
Creating Goals
Opening the Dialog
Click the "+" button to open the Treatment Plan Dialog with empty fields.
Filling Out the Form
- Enter title - Keep concise and action-oriented
- Add description - Be specific about what success looks like
- Specify intervention - Detail the therapeutic approach
- Set target date (optional) - Choose realistic timeframe
- Click save - Goal is created and added to list
After Creation
New goal:
- Appears at top of list (newest first)
- Shows unchecked completion status
- Displays all entered information
- Saved immediately
Editing Goals
Opening Edit Dialog
Click the edit icon on any goal card to modify it:
- Goal dialog opens with pre-filled information
- Edit any field except the ID
- Click save to update
- Changes reflect immediately
Editing Permissions
Only available if:
- You are the note owner
- Usage limits not exceeded
- You have edit permissions
Team members with view-only access cannot edit goals.
Completing Goals
Marking as Complete
Click the checkbox on a goal card to mark it as achieved:
- Checkbox becomes checked
- Status saved automatically
- Goal remains in list
Un-completing Goals
Click a checked checkbox to mark goal as incomplete again:
- Checkbox becomes unchecked
- Status updated automatically
- Useful if client regresses or goal needs continuation
Deleting Goals
Delete Action
Click the delete icon on any goal card:
- Confirmation may appear
- Confirm deletion
- Goal removed from list
Only note owners can delete goals.
Goal Display
Visible Goals (Default)
The first 3 goals display by default, sorted by timestamp (newest first).
Each goal card shows:
- Title prominently displayed
- Description text
- Intervention approach
- Target date (if set)
- Completed checkbox
- Edit and delete icons
Hidden Goals
Goals beyond the first 3 are hidden until you click "Expand All".
Expanded View
When expanded:
- All goals display in chronological order
- Completed and incomplete goals intermixed
- Scroll to see all goals
- Click collapse to return to 3-goal view
Empty State
When no treatment goals exist:
Message displayed: "You have not added any treatment goals for [Client Name] yet."
Actions available:
- Click "+" to create first goal
- Dialog guides you through creation
- First goal appears immediately after saving
Sorting and Organization
Default Sort Order
Goals display by timestamp, newest first:
- Most recently created goal appears first
- Older goals appear lower in list
- Completed status doesn't affect sort order
Filtering
Currently no built-in filter for completed vs. incomplete goals. All goals display together.
Permissions
Note Owner
Full access to:
- Create new goals
- Edit existing goals
- Delete goals
- Mark goals as complete/incomplete
Team Members
Limited to:
- View goals only
- Cannot create, edit, or delete
- Cannot change completion status
Usage Limit Exceeded
When limits exceeded:
- Cannot create new goals
- Cannot edit existing goals
- Can view all goals
- Red banner indicates restriction
Best Practices
Goal Setting
Create effective treatment goals by:
- Using SMART criteria (Specific, Measurable, Achievable, Relevant, Time-bound)
- Collaborating with client on goals
- Aligning goals with client's values
- Starting with 2-3 primary goals
- Adding new goals as initial ones are achieved
Documentation
Document goals thoroughly with:
- Client's own language in description
- Specific measurable criteria
- Evidence-based intervention approaches
- Realistic target dates
- Regular progress reviews
Progress Tracking
Monitor goal progress by:
- Reviewing goals at start of each session
- Updating descriptions as goals evolve
- Marking goals complete when achieved
- Creating new goals to replace completed ones
- Using target dates to maintain momentum
Goal Review
Regularly review treatment plan:
- Assess progress toward each goal
- Modify goals that aren't working
- Add new goals as needs emerge
- Complete goals that have been achieved
- Delete goals no longer relevant
Integration with Other Sections
Session Notes
Reference treatment goals in session notes:
- Document progress toward specific goals
- Note interventions used
- Record client feedback
- Track obstacles or challenges
MSE Assessments
Consider goals when conducting MSE:
- Assess areas related to treatment goals
- Note cognitive function relevant to goals
- Evaluate insight into goal progress
- Document judgment around goal achievement
Client Portal
Share treatment plan with clients via portal:
- Clients can review their goals
- Increases accountability
- Promotes collaboration
- Maintains transparency
